ABSTRACT

Introdução: A foniatria tem interesse em fissura labiopalatina (FLP) devido à complexidade em aspectos de fala, linguagem e audição. Objetivo: No contexto da avaliação foniátrica em crianças escolares com fissura labiopalatina, o objetivo do estudo de múltiplos casos foi descrever a interferência de fatores clínicos no desempenho em testes da função perceptiva auditiva de linguagem, assim como a associação entre eles. Método: Trinta crianças, entre 6 anos e 2 meses e 9 anos e 11 meses, foram avaliadas pelo médico foniatra, em serviço interdisciplinar de tratamento da FLP: 13 (43%) fissura pós-forame incisivo e 17 (57%) fissura transforame incisivo. Foram descritos os fatores clínicos (incluindo tipo de fissura, idade na avaliação, sexo, condição socioeconômica, desempenho escolar, realização de fototerapia, exame audiológico e outros) e o desempenho em testes de discriminação auditiva, memória auditiva e consciência fonológica, incluindo a associação entre eles. Resultados: Não houve diferença significativa entre os grupos de fissura e médias de idade (p=0,618), sexo (p=0,431), nível socioeconômico (p=0,580) e desempenho escolar (p=0,785).A porcentagem de crianças que fez terapia é maior no grupo de fissura transforame (94,1%). Alterações nos testes: 4 crianças (13,3%) discriminação auditiva, 6 (20%) memória auditiva e 8 (26,7%) consciência fonológica. Os casos com desempenho escolar insatisfatório, mais tempo de fonoterapia e piores médias do limiar de reconhecimento de fala tiveram desempenho pior em memória auditiva e consciência fonológica. Conclusão: A avaliação foniátrica possibilitou o apontamento de fatores clínicos da fissura, que parecem, de alguma forma, interferir no desenvolvimento da linguagem destas crianças.


Introduction: Phoniatrics is interested in cleft lip and palate (CLP) due to the complexity in aspects of speech, language and hearing. Objective: In context of phoniatric evaluation in schoolchildren with CLP, the objective of multiple cases study was to describe the interference of clinical factors in the performance in tests of auditory perceptual function of language, as well as the association between them. Method: Thirty children, between 6 years and 2 months and 9 years and 11 months, were evaluated by Phoniatric doctor, in interdisciplinary treatment service for CLP: 13 (43%) cleft palate and 17 (57%) cleft lip and palate. Clinical factors (including type of cleft, age at evaluation, sex, socioeconomic status, school performance, speech therapy, audiological examination and others) and performance in tests of auditory discrimination, auditory memory and phonological awareness were described, including the association between them. Results: There was no significant difference between the cleft groups and the mean age (p = 0.618), sex (p = 0.431), socioeconomic level (p = 0.580) and school performance (p = 0.785). The percentage of children who had attended therapy is higher in cleft lip and palate group (94.1%). Changes in tests: 4 (13.3%) auditory discrimination, 6 (20%) auditory memory and 8 (26.7%) phonological awareness. The cases with unsatisfactory school performance, more speech therapy time and worse speech recognition thresholds had a worse performance in auditory memory and phonological awareness. Conclusion: The phoniatric evaluation made it possible to identify clinical factors of the cleft, which seems, in some way, to interfere in the language development of these children.


Introducción: Foniátrica está interesado en el labio leporino y paladar hendido debido a la complejidad en los aspectos del habla, el lenguaje y la audición. Objetivo: El contexto de la evaluación foniatría en escolares con labio leporino y paladar hendido, el objetivo del estudio de múltiples casos fue describir la interferencia de factores clínicos en el desempeño en las pruebas de la función auditiva perceptiva del lenguaje, así como la asociación entre ellos. Método: Treinta niños, entre 6 años y 2 meses y 9 años y 11 meses de edad, fueron evaluados por un médico foniátrico, en un centro de tratamiento interdisciplinario para labio leporino y paladar hendido: 13 (43%) paladar hendido y 17 (57%) labio leporino y paladar hendido. Se describieron y verificaron los factores clínicos (incluido el tipo de hendidura, la edad en la evaluación, el sexo, el estado socioeconómico, el rendimiento escolar, la fototerapia, el examen audiológico y otros) y el rendimiento en las pruebas de discriminación auditiva, memoria auditiva y conciencia fonológica, incluida la asociación entre ellos. Resultados: No hubo diferencias significativas entre los grupos de hendidura y la edad media (p = 0.618), sexo (p = 0.431), nivel socioeconómico (p = 0.580) y rendimiento escolar (p = 0.785). El porcentaje de niños que se sometieron a terapia es mayor en el grupo de paladar hendido y labio leporino (94.1%). Cambios en las pruebas: 4 niños (13.3%) discriminación auditiva, 6 (20%) memoria auditiva y 8 (26.7%) conciencia fonológica. Los casos con un rendimiento escolar insatisfactorio, más tiempo de terapia del habla y peores umbrales de reconocimiento del habla tuvieron un peor rendimiento en la memoria auditiva y la conciencia fonológica. Conclusión: La evaluación foniátrica permitió identificar factores clínicos de la hendidura, que de alguna manera, parece interferir en el desarrollo del lenguaje de estos niños.

ABSTRACT

A fenda palatina é a comunicação entre a cavidade oral e a nasal através de um orifício no palato. Tem diversas etiologias, podendo ser congênita, traumática, por deficiência mineral ou por fatores hormonais. O diagnóstico é realizado por meio de exame físico da cavidade oral, e a correção cirúrgica é o tratamento de escolha. Em animais adultos, pode ser corrigida com o auxílio de retalho mucoperiosteal, apresentando bons resultados. Já em filhotes, a correção cirúrgica é mais complicada, com prognóstico menos favorável. Este trabalho relata o caso de um canino, fêmea, sem raça definida, adulta, com histórico de fenda palatina secundária, de origem traumática, no palato mole por ingestão de osso. Para a correção cirúrgica, primeiramente foi utilizada membrana biológica de pericárdio bovino, mas não se obteve êxito. O segundo procedimento foi realizado com retalho mucoperiosteal simples autólogo e, dois meses após o procedimento, já havia cicatrização completa. A técnica de retalho mucoperiosteal simples autólogo se mostrou eficaz no tratamento da fenda palatina, aliada aos cuidados adequados no pós-operatório.(AU)


The cleft palate is the communication between the oral and nasal cavity through an aperture in the palate, it's causes include an infinitude of factors: congenital, traumatic, mineral deficiency or hormonal. Examination of the oral cavity determines if the diagnosis and treatment is surgical. Correction in adult animals is performed with mucoperiosteal flap showing good results. However, surgical correction in puppies is more complicated with less favorable prognosis. This current work reports a case of an adult, female dog of undefined breed, with a history of secondary clef palate of traumatic origin in the soft palate due to bone ingestion. For correction, a biological membrane of bovine pericardium was used, but it was not successful, requiring a second surgical procedure performed with autologous simple mucoperiosteal flap. The last technique combined with adequate postoperative care was effective.(AU)

Abstract This study aimed to compare the linear dimensions of the dental arches of adult patients with complete unilateral cleft lip and palate (UCLP) after orthodontic and prosthetic treatment with fixed partial dentures (FPD) to patients without clefts, using 3D technology. This retrospective longitudinal study sample consisted of 35 subjects divided into two groups. Included in this sample were 15 complete UCLP individuals who had received orthodontic treatment before rehabilitation with a fixed partial denture (FG), as well as 20 patients without cleft as control group (CG). All patients were aged between 18 and 30 years. Digital dental casts were obtained in two stages: (T1) end of orthodontic treatment and (T2) one year after prosthetic rehabilitation (FG); and (T1) end of orthodontic treatment and (T2) one year after removal of the orthodontic appliance (CG). Intercanine, interfirst premolar and intermolar distances, and incisor-molar length were obtained. A precalibrated and trained examiner performed the assessments. Intergroup differences between T2 and T1 were compared between the groups using the t test or Mann-Whitney test with a significance level of 5% (p<0.05). The intercanine distance variation (T2-T1) showed statistical difference (p=0.005) increasing in the FG group and decreasing in the CG group. In the interfirst premolar distance variation, FG decreased, while CG increased with statistically significant difference (p=0.008). The intercanine distance of individuals with cleft showed stability, while that of the CG had no stability. The CG showed stability in the interfirst premolar distance, while FG had no stability. These findings showed that the FPD is capable of restricting orthodontic results, leading to a stabilization of the dental arches.

Las fisuras labiopalatinas (FLP) son las malformaciones craneofaciales congénitas más comunes. La anemia de Diamond-Blackfan o anemia aneritroblástica es una hipoplasia congénita selectiva de la serie roja, asociada a alteraciones cardiacas, renales, malformaciones en manos, hipertelorismo y retraso en el crecimiento. La presentación de las FLP y pacientes con Anemia de Diamond-Blackfan es del 10.3%. Objetivo: Presentar un reporte de caso de un paciente de 12 años con anemia de Diamond-Blackfan y secuela de FLP de la Clínica Interdisciplinaria de LPH de la Pontificia Universidad Javeriana (Bogotá-Colombia) donde el abordaje interdisciplinario, el casomanejo del comportamiento y el apoyo psicológico brindado a la familia fueron fundamentales para lograr la adherencia, clarificar el diagnóstico y obtener los resultados terapéuticos esperados. Resultados: se logró la adherencia al tratamiento y el éxito de éste en apoyo con otras especialidades, permitiendo un seguimiento de 2 años. Conclusiones: El manejo multidisciplinario e interdisciplinario con especialidades odontológicas, médicas, y de la salud en general para el manejo de pacientes con FLP y síndromes asociados, resulta importante para el éxito en el tratamiento.

A anemia do Diamante-Blackfan ou anemia aneritroblástica seletiva é uma hipoplasia congênita da série vermelha, associada com o coração, rim, malformações mãos, hipertelorismo e distúrbios retardo de crescimento. A apresentação de Fissura labiopalatina e pacientes com anemia do Diamante-Blackfan é de 10,3% dos casos. Objetivo: Apresentar um caso clínico de um paciente de 12 anos com anemia do Diamante-Blackfan e Fissura labiopalatina sequela do Interdisciplinar Clinic LPH da Pontificia Universidad Javeriana (Bogotá Colômbia), onde a abordagem interdisciplinar, gestão de comportamento e apoio psicológico prestado aos família foram fundamentais para alcançar a adesão a esclarecer o diagnóstico e obter os resultados terapêuticos esperados. Resultado: a adesão ao tratamento e seu sucesso no apoio de outras especialidades de saúde alcançados. Conclusões: O tratamento multidisciplinar e interdisciplinar com especialidades odontológicas, médicas e de saúde em geral para o tratamento de pacientes com FLP e síndromes associadas, é importante para o sucesso do tratamento


Cleft lip and palate (CLP) are the most common congenital craniofacial malformations. The Diamond-Blackfan anemia or congenital erythroblastic anemia is a selective red cell hypoplasia associated with heart, kidney, malformed hands, hypertelorism and stunted alterations. The presentation of Cleft lip and palate in patients with Diamond-Blackfan anemia is 10.3% of cases. Aim: To present a case report of a patient of 12 years with Diamond-Blackfan anemia and Cleft lip and palate sequel from the Interdisciplinary Clinic LPH of the Pontificia Universidad Javeriana (Bogota Colombia) where the interdisciplinary approach, behavior management and psychological support provided to the family were instrumental in achieving the expected therapeutic results. Results: Adherence to treatment and its success in supporting other health specialties during two years is slown. Conclusions: The multidisciplinary and interdisciplinary management with dental specialties, medical, and health in general for the management of patients with FLP and associated syndromes, it is important for successful treatment

Introdução: As fissuras labiopalatinas não sindrômicas estão incluídas entre as malformações craniofaciais severas mais frequentes na população humana. O indivíduo acometido pode apresentar alterações no mecanismo velofaríngeo, comprometendo-se assim a inteligibilidade da fala. O objetivo deste estudo foi analisar o efeito imediato do exercício de trato vocal semiocluído no mecanismo velofaríngeo de pacientes com fissura labiopalatina. Métodos: A amostra foi composta por cinco sujeitos com fissura labiopalatina reparada cirurgicamente submetidos a avaliação multidimensional, de videonasofibroscopia, análise das imagens dinâmicas por meio de software especializado e análise de fala por meio da avaliação perceptivoauditiva e parecer autorreferido. Resultados: Todos os pacientes apresentaram melhora na fala encadeada após o exercício, melhora acentuada na impressão geral da emissão e projeção vocal, maior fechamento velofaríngeo e efeito positivo na autoavaliação. Conclusão: Observou-se efeito positivo do exercício de trato vocal semiocluído quando aplicado nessa amostra.

Abstract The impact of oral rehabilitation on masticatory function and oral health-related quality of life (OHRQoL) may vary with the experience of the individual with tissue loss. Our hypothesis is that patient-centered outcomes vary among adults who have experienced large defects in the maxilla due to congenital or acquired conditions even after oral rehabilitation to restore aesthetics and function. This study compared OHRQoL, perceived masticatory ability, maximum bite force (MBF), and symptoms of pain and depression among subjects with acquired (edentulous maxilla) and congenital (cleft lip and palate) loss of oral tissues in the maxilla after dental treatment. A gender-matched sample (n = 60) of cleft lip and palate (CLP), maxillary denture wearers (DENT) and controls (CONT) was recruited. OHRQoL was assessed using OHIP-14. Chewing was evaluated through a masticatory ability questionnaire and by MBF. The RDC/TMD Axis II questionnaire was used to assess symptoms of pain and depression. Data were analyzed by Fisher's test, Kruskal Wallis test, and Spearman correlation coefficients. CLP showed higher OHIP-14 and depression scores than DENT and CONT (p < 0.05). Sub-analysis by OHIP-14 items (%FOVO) showed higher prevalence of psychological impact for CLP and of functional impacts for DENT. The number of foods difficult to chew, of food textures difficult to chew, and avoided foods were similar between CLP and DENT. OHIP-14, MBF, and depression scores showed significant correlation (p < 0.05). The results suggest that adults with treated CLP or maxillary DENT have chewing impairment and lower MBF than healthy subjects, with different psychological and functional impacts.

Abstract Objective This study analyzed the maintenance of lateral incisors in the dental rehabilitation of individuals with cleft lip and palate. Material and Methods The study was conducted on a tertiary craniofacial center and comprised retrospective analysis of panoramic and periapical radiographs of Caucasoid individuals with non-syndromic complete unilateral cleft lip and palate, analyzing all radiographs available on the records of each individual, from the first to the last up to 12 years of age. Overall, 2,826 records were reviewed to achieve a sample of 1,000 individuals. Among these, 487 individuals presented the permanent lateral incisors on both cleft and non-cleft sides, which were included in this study. Results The results were evaluated in percentages and by descriptive statistics. The association between maintenance of the lateral incisor and timing of alveolar bone graft were analyzed by the t test. Among the 487 individuals, 265 had not completed treatment, 62 presented insufficient information, and 44 concluded the treatment elsewhere. Among the remaining 116 individuals, the lateral incisor was extracted from 88 (75.86%) of them on the cleft side (CS) and from 23 (19.83%) people on the non-cleft side (NCS). The age at accomplishment of alveolar bone graft was significantly associated with maintenance of the lateral incisor on the cleft side (p<0.01). Most extractions were indicated because of the inadequate positioning on the CS and for midline correction on the NCS. Rehabilitation was primarily completed by orthodontic movement (53 individuals on the CS and 13 individuals on the NCS). Conclusion In conclusion, the lateral incisor on the cleft side was not maintained in most individuals. Positive relationship was observed between extraction of the lateral incisor and age at accomplishment of the alveolar bone graft, suggesting the need to anticipate the initial radiographic evaluation to enhance its maintenance and reduce the procedures required for rehabilitation.

ABSTRACT

RESUMEN: La fisura labio palatina (FLP) es una alteración del desarrollo, congénita, de etiología desconocida. La fístula oronasal es la complicación más común de la reparación del paladar con fisura. Los problemas más comunes incluyen hipernasalidad al hablar y el pase de fluidos y comida hacia la cavidad nasal. El caso corresponde a un paciente de 16 años de edad, sexo masculino, con fístula buconasal de 2,1 cm de diámetro mayor en sentido transversal, como secuela de FLP unilateral derecha. Además relata problemas en la alimentación, traspaso de fluidos a la cavidad nasal, problemas de habla por insuficiencia velo faríngea (IVF) valor 9, alteraciones en sus relaciones interpersonales y baja autoestima. Se realizó el injerto de lengua en fístula buconasal, tratamiento de ortodoncia y rehabilitación oral para dar solución estética y funcional. El éxito del tratamiento integral le permitió al paciente mejorar sus relaciones sociales debido a una mayor autoestima.


ABSTRACT: The cleft lip and palate (CLP) is a developmental and congenital anomaly of unknown etiology. The oronasal fistula is the most common complication of the cleft palate reparation. The most common problems are hypernasality on speech and the passage of fluids and food to the nasal cavity. This case is about a sixteen- year-old boy with a 2,1 cm diameter oronasal fistula due to a right unilateral CLP sequel. He also presented alimentation problems, fluid passage to the nasal cavity, speech defects by velopharyngeal insufficiency (VFI), relationships problems and low self-esteem. A tongue graft in the oronasal fistula, orthodontic treatment and oral rehabilitation were made to give an aesthetic and functional solution. The success of the treatment allowed the patient to improve his relationships due to a greater self-esteem.

RESUMEN: El paciente fisurado labiopalatino muestra una falta de fusión de los procesos palatinos presentando una comunicación de la cavidad buconasal, anomalías de forma y/o la agenesia de los dientes cercanos a la fisura y un deficiente crecimiento sagital y transversal maxilar, esto es más complejo en la fisura bilateral, por ser el más severo de los subtipos de fisura. Se dificultan las funciones básicas como comer, hablar y relacionarse con los demás. Su tratamiento requiere del trabajo de un equipo transdisciplinario, coordinado y organizado, conformado por todos aquellos profesionales que colaboren en proporcionar un resultado integral. El objetivo de este artículo es exponer un caso clínico de rehabilitación protésica en conjunto con un tratamiento de ortodoncia en un paciente que presenta labio leporino bilateral y fisura palatina, además de una premaxila móvil, severamente extruida.


ABSTRACT: The cleft lip and palate patient is mainly characterised by the presence of an oronasal communication, malformation or agenesis of the teeth close to the cleft, and deficient sagittal and transverse growth of the maxilla. These patients require various treatments involving a multidisciplinary team, which may include a maxillofacial surgeon, an orthodontist, a speech therapist, a paediatrician, plastic surgeon, a general dentist, a prosthodontist, an ENT specialist, a psychologist, and all those professionals who can help provide functional, aesthetic and psychological improvement. This report describes a case of prosthetic rehabilitation in a patient with cleft lip and palate without alveolar bone in the cleft area, which prevented rehabilitation with an osseointegrated implant, in addition to a mobile and severely extruded pre-maxilla, which required an orthopaedic repositioning.

Abstract The objective of this study was to investigate the prevalence of live births with orofacial clefts in Brazil from 2009 to 2013, according to Brazil’s federative units and regions, and correlate it with the number of corrective surgery procedures for cleft lip and palate performed through the Brazilian National Health System in the same period. The data were collected from the National Live Birth System (SINASC in Portuguese) and the Hospital Information System (SIH-SUS in Portuguese). The results showed that the average prevalence of oral clefts in the period was 5.86 per 10,000 live births, with differences observed between the federative units and the regions. The correlation between the prevalence of orofacial clefts and the number of medical procedures associated with this anomaly was statistically significant (r=0.94; p <0.001). This result indicates that greater investments are being made in areas with a higher number of cases. The relationship between supply and demand for corrective surgeries suggests that only 18.91% of the live births with orofacial clefts in this period may have received this service under the Brazilian National Health System.

Abstract This study aimed at monitoring the maxillary growth of children with cleft lip/palate in the first two years of life, and to evaluate the effects of primary surgeries on dental arch dimensions. The sample consisted of the three-dimensional digital models of 25 subjects with unilateral complete cleft lip and palate (UCLP) and 29 subjects with isolated cleft palate (CP). Maxillary arch dimensions were measured at 3 months (before lip repair), 1 year (before palate repair), and at 2 years of age. Student’s ttest was used for comparison between the groups. Repeated measures ANOVA followed by Tukey’s test was used to compare different treatment phases in the UCLP group. Paired ttest was used to compare different treatment phases in the CP group. P<0.05 was considered statistically significant. Decreased intercanine distance and anterior arch length were observed after lip repair in UCLP. After palate repair, maxillary dimensions increased significantly, except for the intercanine distance in UCLP and the intertuberosity distance in both groups. At the time of palate repair and at two years of age, the maxillary dimensions were very similar in both groups. It can be concluded that the maxillary arches of children with UCLP and CP changed as a result of primary surgery.

RESUMO Introdução Na fissura de palato, a realização de um diagnóstico preciso das alterações de fala auxilia o processo de reabilitação, direcionando o tratamento para a disfunção velofaríngea. Objetivo Determinar o estímulo de fala que melhor identifica a hipernasalidade, comparando a nasalidade da fala e a nasalância em estímulos de alta e baixa pressão intraoral, em indivíduos com fissura palatina. Métodos Quarenta e quatro indivíduos com fissura de palato±lábio operada, de ambos os sexos, com idades entre 6 e 59 anos, foram submetidos, simultaneamente, à nasometria e gravação de fala. A nasalância foi determinada utilizando amostras de fala com consoantes de alta pressão intraoral (AP) e consoantes de baixa pressão intraoral (BP). Três examinadores experientes classificaram a nasalidade nas duas amostras (AP e BP), de acordo com uma escala de 4 pontos (1=hipernasalidade ausente, 2=hipernasalidade leve, 3=hipernasalidade moderada, 4=hipernasalidade grave). Resultados Os escores médios ±desvio padrão de nasalância obtidos para as amostras AP e BP foram 31±15% e 31±12%, respectivamente, sem diferença (p=1,0). A concordância entre os examinadores foi maior para as amostras AP. A média de nasalidade entre as duas amostras apresentou diferença (p=0,05). As amostras AP apresentaram forte correlação entre os escores de nasalância e hipernasalidade e as amostras BP apresentaram correlação substancial. Conclusão A amostra composta por consoantes de alta pressão intraoral mostrou maior eficácia na identificação da hipernasalidade, pois proporcionou maior concordância entre os examinadores na análise perceptual da nasalidade, apresentou forte correlação entre os dois métodos utilizados e permitiu o diagnóstico de hipernasalidade em maior número de indivíduos.


ABSTRACT Introduction In the cleft palate, the accurate diagnosis of speech disorders helps the rehabilitation process by directing the treatment of velopharyngeal dysfunction. Purpose To verify the influence of pressure consonants on speech nasality and nasalance, comparing high and low intraoral pressure stimuli in individuals with cleft palate. Methods Forty-four subjects with repaired cleft palate±lip, both genders, aged 6 to 59 years were simultaneously submitted to nasometry and audio speech sample recording. Nasalance scores were determined for speech samples with high-pressure consonants (HP) and low-pressure consonants (LP). Three experienced raters classified speech nasality in both samples (HP and LP) according to a 4-point scale (1 = absent hypernasality, 2 = mild hypernasality, 3 = moderate hypernasality, 4 = severe hypernasality). Results Nasalance scores±SD obtained for HP and LP samples were 31±15% and 31±12%, respectively, with no difference (p=1.0). The inter-rater agreement was higher for HP sample. The average rate of nasality between both samples showed difference (p=0.05). HP samples presented strong correlation between nasalance scores and hypernasality and LP samples presented substantial correlation. Conclusion The HP speech sample was shown to be more effective in identifying hypernasality, as it provided greater agreement among examiners in the perceived nasality analysis, had a strong correlation between the two methods used and allowed diagnosis of velopharyngeal disfunction in a larger number of individuals.

ABSTRACT

La fisura labiopalatina consiste en la falta de fusión de los procesos palatinos y labial durante el desarrollo embrionario. Estos pacientes presentan una comunicación buconasal, alteraciones dentarias de forma, tamaño y posición, y un deficiente desarrollo del maxilar. Su tratamiento requiere un trabajo de equipo multidisciplinar, ya que afecta funciones básicas que comprometen su calidad de vida, como comer, hablar o relacionarse socialmente. Aunque la cirugía primaria soluciona mayormente esta anomalía, algunas veces persiste la comunicación entre las cavidades bucal y nasal, lo que se conoce como comunicación o fístula buconasal. Esto supone una mayor dificultad en el cierre quirúrgico, siendo a veces imposible de cerrar a través de la cirugía. En tales casos, la obturación de dicha fístula a través de una prótesis otorga una solución de bajo costo para el paciente y con resultados óptimos a corto y mediano plazo. El objetivo de este artículo es exponer casos de rehabilitación protésica en pacientes con fisura labiopalatina y fístula buconasal posterior al tratamiento quirúrgico.


Cleft lip and palate is the lack of fusion of the palatal and labial processes during embryonic development. Patients are characterised by the presence of buconasal communication, dental alterations, and poor development of the maxilla. Treatment requires multidisciplinary team work, due to the impact on basic functions such as eating, talking or interacting with other people, and even compromising their quality of life. Although primary surgery mostly solves this anomaly, sometimes communication between the oral and nasal cavity persists (called communication or buconasal fistula). This leads to greater difficulty to surgically close it, being impossible in some cases. In these cases, the closure using a prosthesis provides a low-cost solution for the patient, with an optimal result in the short and medium term. The aim of this article is to present cases of prosthetic rehabilitation in patients with cleft lip and buconasal fistula after surgical treatment.

RESUMO Objetivo Investigar a influência do tipo de amostra de fala, conversa espontânea ou repetição de sentenças, sobre o índice de concordância intra e interavaliadores obtido na classificação perceptiva da hipernasalidade. Métodos Foram selecionadas e editadas 120 amostras de fala gravadas em áudio (60 contendo trechos de conversa espontânea e 60 contendo repetição de sentenças) de indivíduos com fissura de palato±lábio reparada, de ambos os sexos, com idade entre 6 e 52 anos (média=21±10 anos). Três fonoaudiólogas experientes, utilizando seus critérios internos, classificaram a hipernasalidade em escala de 4 pontos: 1=ausente, 2=leve, 3=moderada e 4=grave, primeiramente na amostra de conversa espontânea e, 30 dias depois, na repetição de sentenças. Os índices de concordância intra e interavaliadores foram estabelecidos para ambos os tipos de amostra de fala e comparados entre si por meio do Teste Z com nível de significância de 5%. Resultados A comparação dos índices de concordância intra-avaliadores entre os dois tipos de amostra de fala mostrou aumento dos coeficientes obtidos na análise da repetição de sentenças em relação aos obtidos na conversa espontânea, já a comparação entre os índices de concordância interavaliadores não mostrou diferença significante entre as três avaliadoras para os dois tipos de amostras de fala. Conclusão A repetição de sentenças favoreceu a confiabilidade do julgamento perceptivo da hipernasalidade de um mesmo avaliador, visto que a concordância intra-avaliadores na análise desta amostra de fala foi maior. No entanto, o tipo de amostra de fala não influenciou a concordância entre diferentes avaliadores.


ABSTRACT Purpose To investigate the influence of speech sample of spontaneous conversation or sentences repetition on intra and inter-rater hypernasality reliability. Methods One hundred and twenty audio recorded speech samples (60 containing spontaneous conversation and 60 containing repeated sentences) of individuals with repaired cleft palate±lip, both genders, aged between 6 and 52 years old (mean=21±10) were selected and edited. Three experienced speech and language pathologists rated hypernasality according to their own criteria using 4-point scale: 1=absence of hypernasality, 2=mild hypernasality, 3=moderate hypernasality and 4=severe hypernasality, first in spontaneous speech samples and 30 days after, in sentences repetition samples. Intra- and inter-rater agreements were calculated for both speech samples and were statistically compared by the Z test at a significance level of 5%. Results Comparison of intra-rater agreements between both speech samples showed an increase of the coefficients obtained in the analysis of sentences repetition compared to those obtained in spontaneous conversation. Comparison between inter-rater agreement showed no significant difference among the three raters for the two speech samples. Conclusion Sentences repetition improved intra-raters reliability of perceptual judgment of hypernasality. However, the speech sample had no influence on reliability among different raters.

ABSTRACT

Objetivo Identificar as repercussões sociais em indivíduos com distúrbios da comunicação associados às fissuras labiopalatinas com e sem perda auditiva. Métodos Participaram crianças e adolescentes de 7 anos a 15 anos e 11 meses de idade, de ambos os gêneros, com fissuras de lábio e palato ou de palato isolado, com e sem perda auditiva associada. A pesquisa foi composta por 52 participantes, divididos em dois grupos: um constituído por 36 crianças e adolescentes com fissuras labiopalatinas e sem perda auditiva e outro, por 16 sujeitos com fissuras labiopalatinas e com perda auditiva associada. Resultados Constatou-se que as repercussões socioeconômicas, familiares, educacionais e sociais são comuns aos grupos. Conclusão As crianças e adolescentes com perda auditiva associada à fissura labiopalatina não estão em “desvantagem” no que se refere às repercussões econômicas, familiares, escolares e sociais, em relação às que não têm o distúrbio de audição. Ambos os grupos vivenciam o acirramento de conviver com o comprometimento estético e funcional causado pela anomalia e de viver em uma sociedade totalmente preocupada com a imagem e julgadora das diferenças. .


Purpose To identify the social impact on individuals with communication disorders associated with cleft lip and palate with and without hearing loss. Methods Participation by children and adolescents from 7 years to 15 years and 11 months old, of both genders, with cleft lip and palate or isolated cleft palate, with and without associated hearing loss. The survey consisted of 52 participants, divided into two groups: one consisting of 36 children and adolescents with cleft lip and palate without hearing loss and the other, 16 subjects with cleft lip and palate with associated hearing loss. Results It was found that the socio-economic, family, educational and social effects are common to both groups. Conclusion Children and adolescents with hearing loss associated with cleft lip and palate are not “disadvantaged” when referring to economic, family, school and social repercussions in relation to those who do not have hearing disorders. Both groups experience the worsening of living with the aesthetic and functional impairment caused by the anomaly and living in a society entirely concerned with the image and judging differences. .

ABSTRACT

Purpose: This study obtained nasalance scores during use of compensatory articulation (CA) and compared nasalance between groups with and without hypernasality and with and without CA. Methods: Speech samples were obtained from 43 individuals with and without velopharyngeal dysfunction during repetition of 20 phrases originating 860 audio recordings and their respective nasometric values. After excluding 143 recordings due to low quality, the remaining 717 samples were rated by three speech language pathologists (SLPs), independently, for presence or absence of hypernasality and CA. Nasalance scores for the 553 samples rated with 100% agreement among the SLPs were grouped according to the auditory-perceptual ratings: Group 1 (G1) - included samples without hypernasality and without CA (n=191); Group2 (G2) - included samples with hypernasality and without CA (n=288); Group 3 (G3) - included samples with hypernasality and with pharyngeal fricative (n=33); Group 4 (G4) - included samples with hypernasality and with glottal stop (n=41). Results: Analysis of variance (ANOVA) revealed significant difference nasalance scores which were significantly higher for G2, G3, and G4 (p<0.0001) when compared to G1. The use of pharyngeal fricative (G3), particularly during /f/ (p=0.0018) and /s/ (p=0.0017) productions resulted in nasalance scores significantly higher than scores found for G2. Conclusion: Significantly higher nasalance values where identified during use of pharyngeal fricative.


Objetivo: Este estudo obteve medidas de nasalância durante a produção de articulação compensatória (AC) e comparou a nasalância entre grupos com e sem hipernasalidade e com e sem AC. Métodos: As amostras de fala foram obtidas a partir de 43 indivíduos com e sem disfunção velofaríngea durante a repetição de 20 frases, originando um total de 860 gravações e respectivos valores nasométricos. Foram excluídas 143 gravações devido à baixa qualidade e as 717 amostras restantes foram avaliadas por três fonoaudiólogas, de forma independente, quanto à presença ou ausência de hipernasalidade e AC. As 553 amostras julgadas com 100% de concordância entre as fonoaudiólogas foram agrupadas de acordo com o julgamento perceptivo-auditivo: Grupo 1 (G1) - amostras sem hipernasalidade e sem AC (n=191); Grupo 2 (G2) - amostras com hipernasalidade e sem AC (n=288); Grupo 3 (G3) - amostras com hipernasalidade e com fricativa faríngea (n=33); Grupo 4 (G4) - amostras com hipernasalidade e com oclusiva glotal (n=41). Resultados: Análise de variância (ANOVA) revelou medidas de nasalância significativamente maiores para G2, G3 e G4 (p<0,0001) quando comparados ao G1. O uso de fricativa faríngea (G3), particularmente durante /f/ (p=0,0018) e /s/ (p=0,0017) resultou em valores de nasalância significativamente maiores que os valores encontrados para G2. Conclusão: Valores de nasalância significativamente mais elevados foram encontrados durante produção de fricativa faríngea.

ABSTRACT

As fissuras labiopalatinas levam os profissionais a idealizarem próteses não convencionais na busca da solução das reabilitações por se tornarem, muitas vezes, bastante complexas. Próteses totalmente implanto-suportadas devem ser o tratamento de escolha, embora haja casos em que a união de dentes com implantes é inevitável. Há autores que defendem a união rígida, outros a não rígida e outros ainda que não recomendem a união dente-implante. Os riscos relacionados a esta união são resultado de diferenças biomecânicas entre as estruturas envolvidas, ligamento periodontal e osso, e da biomecânica dos dentes e implantes apoiados sobre as próteses. Objetivo: este trabalho relata um caso clínico de reabilitação oral com uma PPR sobre dentes unidos a implantes em uma paciente com fissura labiopalatina do Hospital de Reabilitação de Anomalias Craniofaciais de Bauru. Método: relato do caso. Resultados e Discussão: paciente de 47 anos, gênero feminino, portadora de fissura pós-forame incisivo incompleta, apresentava os dentes 13, 23 e 27, sendo este extraído por problemas periodontais, e 2 implantes instalados nas regiões dos dentes 16 e 26. O planejamento reabilitador protético foi a instalação de uma PPR com encaixe tipo barra-clip retida por 3 barras metálicas unindo os dentes aos implantes por meio de um sistema tubo parafuso. A união não-rígida demonstra maior distribuição das forças, embora possa causar a intrusão dentária. Já a união rígida, apresenta maior estresse e maior perda óssea na região dos implantes. Conclusão: o correto planejamento, embasado na biomecânica, assim como controles clínicos e rádio gráficos posteriores, são de suma importância para o sucesso do tratamento reabilitador protético...


Cleft lip and palate lead professionals to idealize unconventional prostheses to find the solution for the rehabilitation that becomes, oftentimes, complex. Totally implant-supported prostheses should be the treatment of choice, although there are cases in which the connection of teeth with implants is inevitable. Some authors advocate the rigid connection, others the non-rigid connection and still others do not recommend the implant-tooth connection. The risks related to this connection are the result of biomechanical differences between the structures involved, periodontal ligament and bone, and biomechanics of teeth and implants supported on the prostheses. Objective: this report presents a case of oral rehabilitation with a removable partial prosthesis on teeth connected to implants in a patient with cleft lip and palate in Hospital of Rehabilitation of Craniofacial Anomalies of Bauru. Method: case report. Results and Discussion: a 47 year old female patient, with incomplete post foramen cleft presenting the teeth 13, 23 and 27, which was extracted by periodontal problems, and two implants placed in regions of the teeth 16 and 26. The prosthetic rehabilitation treatment was the installation of a removable partial prosthesis with a bar-clip type of fit retained by 3 metal bars connecting teeth to implants through a tube screw system. ABSTRACT

The Laboratory of Physiology provides support for the diagnosis of functional disorders associated with cleft lip and palate and also conducts studies to assess, objectively, the institutional outcomes, as recommended by the World Health Organization. The Laboratory is conceptually divided into three units, namely the Unit for Upper Airway Studies, Unit for Stomatognathic System Studies and the Unit for Sleep Studies, which aims at analyzing the impact of different surgical and dental procedures on the upper airways, stomatognathic system and the quality of sleep of individuals with cleft lip and palate. This paper describes the main goals of the Laboratory in the assessment of procedures which constitute the basis of the rehabilitation of cleft lip and palate, i.e., Plastic Surgery, Orthodontics and Maxillofacial Surgery and Speech Pathology.
